What Are Cheek Implants?

Cheek implants are solid, biocompatible implants placed over the cheekbone to add volume, projection, and definition to the midface. Full cheeks and defined cheekbones help create a balanced, aesthetically pleasing face, and patients who have lost facial volume with aging or who were born with weak or flat cheeks can benefit from this enhancement. The implants are made of solid silicone or other biocompatible materials and are selected to suit each individual patient's facial contour. Because the implant is a stable, solid material, it provides long-lasting enhancement without the routine maintenance that injectable fillers require. Implants come in different shapes and positions to address different concerns. Malar implants sit higher on the cheekbone to create more angular, defined structure, while submalar implants sit lower in the midface to restore fullness to a flat or sunken area. Some patients benefit from a combination of both. Dr. Kurtzman evaluates your facial proportions and bone structure to determine the implant style and position that best complements your features. Cheek implants are frequently performed in conjunction with other facial procedures such as a facelift, eyelid surgery, or rhinoplasty, allowing several areas of the face to be addressed together for a harmonious overall result.

Technique Options

Dr. Kurtzman offers multiple facelift techniques tailored to your unique needs and goals:

Malar Implants

Implants positioned higher on the cheekbone to add projection and create more angular, defined cheekbone structure across the upper midface.

Best for: Patients who want more definition and a higher, sculpted cheekbone appearance.

Submalar Implants

Implants positioned lower in the midface to restore fullness to a flat or sunken area below the cheekbone, often helpful where volume has been lost with aging.

Best for: Patients with a hollow or flat midface who want to restore fullness rather than add cheekbone definition.

Combined Malar-Submalar Augmentation

A combination approach that addresses both the cheekbone and the midface to add definition and fullness together for a balanced result.

Best for: Patients who want both added projection over the cheekbone and restored volume in the midface.

Ideal Candidates

Cheek implants can benefit adults who want fuller, more defined cheeks. During consultation, we evaluate your facial structure, discuss your goals, and determine whether implants are the right approach for you:

  • Have flat, weak, or underdeveloped cheekbones that you have always been self-conscious about
  • Have lost midface volume with aging and want to restore fullness in a lasting way
  • Want more definition and structure across the cheekbones for better facial balance
  • Prefer a long-lasting result rather than the ongoing maintenance that injectable fillers require
  • Are already considering a facelift, eyelid surgery, or rhinoplasty and want to enhance the cheeks at the same time
  • Are in good overall health and do not smoke, which supports proper healing
  • Have realistic expectations about what cheek enhancement can achieve for your face

The Procedure

Cheek implant surgery is a relatively short procedure that places a solid implant over the cheekbone through a hidden incision. The approach is tailored to your anatomy and goals:

1. Evaluation & Implant Selection

Dr. Kurtzman analyzes your facial proportions and cheek structure to determine the implant style (malar, submalar, or a combination), size, and position that will best enhance your face. The implant is chosen to match your individual contour.

2. Anesthesia

The procedure can be performed under sedation or general anesthesia depending on your needs and whether it is combined with other surgery. Your comfort and safety are the priority throughout.

3. Incision Placement

A small incision is typically made inside the mouth, along the upper gum line, so there is no visible external scar. In some cases an incision in the lower eyelid may be used instead. The approach is selected to keep scarring hidden.

4. Pocket Creation

A precise pocket is created over the cheekbone, sized to fit the implant closely. A snug pocket helps hold the implant in proper position and limits the chance of shifting.

5. Implant Placement & Fixation

The implant is positioned over the cheekbone and secured to bone or surrounding soft tissue so it stays stable over time. Placement is checked for symmetry between the two sides before closing.

6. Closure

The incision is closed with sutures. Because the incision is inside the mouth in most cases, healing is straightforward and there is no external scar to manage.

Recovery & Results

Recovery from cheek implant surgery is usually manageable, though the midface tends to swell more visibly than some other areas. First few days: Expect soreness, swelling, and some numbness around the cheeks and incision. If the incision is inside the mouth, a soft diet and gentle oral care are recommended while the area heals. Most discomfort is well controlled with prescribed or over-the-counter medication. First 1-2 weeks: Many patients feel ready to return to work and normal daily activities within a week or so, though noticeable swelling may still be present. Strenuous exercise and heavy lifting should be avoided for several weeks. Weeks to one month: Swelling continues to resolve gradually, and numbness around the cheeks improves over time. Avoid pressure or impact to the cheeks while healing.

You will notice added cheek volume right away, and the result continues to improve as swelling subsides over the following weeks. Once healing is complete, cheek implants provide long-lasting enhancement and, unlike injectable fillers, do not require routine touch-up treatments to maintain the result. The implant becomes a stable part of your facial contour.

Frequently Asked Questions

What are cheek implants made of?

Cheek implants are solid implants made of silicone or other biocompatible materials. They are selected to suit your individual facial contour and are placed over the cheekbone, where they add lasting volume and definition to the midface.

Where are the incisions, and will I have a visible scar?

In most cases the incision is made inside the mouth along the upper gum line, so there is no visible external scar. In some situations an incision in the lower eyelid may be used instead. The approach is chosen to keep any scarring well hidden.

How long do cheek implants last?

Cheek implants are designed to be a long-lasting enhancement. Because the implant is a stable, solid material secured over the cheekbone, it does not require the routine maintenance that injectable fillers need. The implant stays in place as a lasting part of your facial contour.

How are cheek implants different from fillers?

Injectable fillers add temporary volume and gradually break down, so they require repeat treatments to maintain the result. Cheek implants provide a single, long-lasting enhancement using a solid implant. Fillers can be a good option for previewing added volume or for those who prefer a non-surgical approach, while implants suit patients wanting a more permanent change.

Can cheek implants be combined with other procedures?

Yes. Many patients have cheek implants in conjunction with another facial procedure such as a facelift, eyelid surgery, or rhinoplasty. Addressing several areas together can create a more balanced and harmonious overall result.

How long does the procedure take?

Cheek implant surgery is relatively short, typically taking about 30 minutes to an hour, though the exact time depends on your anatomy and whether it is combined with other procedures. It is usually performed on an outpatient basis.
