What is a Brow Lift?
A brow lift (forehead lift) is a surgical procedure that elevates sagging eyebrows and reduces forehead wrinkles to restore a more youthful, alert, and approachable appearance. As we age, gravity, sun damage, and repeated muscle movements cause the brow to descend, creating a tired, angry, or sad expression that doesn't reflect how you actually feel. Heavy brows can also contribute to upper eyelid hooding, making you look older than your years. A brow lift repositions the brows to their natural, youthful height, typically at or just above the bony orbital rim, while simultaneously smoothing horizontal forehead lines and reducing vertical frown lines between the brows. Dr. Kurtzman offers both traditional and minimally invasive endoscopic techniques, which produce similar results; the endoscopic approach uses small, hidden incisions with minimal scarring. The procedure is often combined with blepharoplasty for full upper face rejuvenation, as addressing both the brows and eyelids creates a harmonious, natural-looking result that neither procedure alone can achieve.
Technique Options
Dr. Kurtzman offers multiple facelift techniques tailored to your unique needs and goals:
Endoscopic Brow Lift
The most commonly performed technique using small incisions within the hairline and a tiny camera for visualization. Provides excellent lift with minimal scarring and faster recovery.
Best for: Most patients, especially those with good hairline and moderate brow descent
Temporal (Lateral) Brow Lift
Focuses on lifting the outer portion of the brow through small incisions in the temple area. More limited lift but shorter recovery.
Best for: Patients whose main concern is outer brow descent and lateral hooding
Direct Brow Lift
Incision placed directly above the eyebrow, removing a strip of skin. Most powerful lift but visible scar. Reserved for select cases.
Best for: Older patients with deep forehead wrinkles, thick eyebrows that hide scars, or medical limitations
Combined Brow Lift with Blepharoplasty
Brow lift performed together with upper eyelid surgery for full upper face rejuvenation. Addresses both brow position and excess eyelid skin.
Best for: Patients with both brow descent and excess upper eyelid skin who want complete upper face transformation
Ideal Candidates
Brow lift surgery delivers excellent results for patients experiencing age-related brow descent. During consultation, Dr. Kurtzman will evaluate your facial anatomy and discuss whether brow lift addresses your concerns:
- Sagging or low-positioned eyebrows that make you look tired, angry, or sad
- Heavy brows that contribute to upper eyelid hooding or obstruct peripheral vision
- Deep horizontal creases across the forehead that persist at rest
- Vertical frown lines (glabellar lines) between the eyebrows that create an angry appearance
- Brow asymmetry where one brow sits noticeably higher than the other
- Good overall health without conditions that impair healing
- Non-smoker or willing to quit for optimal healing
- Realistic expectations, understanding that brow lift refreshes but doesn't stop aging
The Procedure
Brow lift can be performed using a traditional or an endoscopic approach; both produce similar results. The endoscopic technique described here uses smaller incisions with minimal scarring:
1. Anesthesia & Preparation
Brow lift is performed under general anesthesia or IV sedation combined with local anesthesia for your comfort. The forehead and scalp are prepared, and planned incision sites are marked within the hairline.
2. Endoscopic Incisions
Several small (1-2cm) incisions are made within the hairline. These heal to become virtually invisible. An endoscope (small camera) is inserted to visualize the underlying structures.
3. Tissue Release & Elevation
The forehead skin and underlying tissues are gently released from the bone and lifted to a higher position. This releases the downward pull of gravity and elevates the brows.
4. Muscle Treatment
The corrugator and procerus muscles that create frown lines can be weakened or partially removed to reduce vertical lines between the brows and provide longer-lasting Botox-like effects.
5. Fixation
The elevated tissues are secured in their new position while healing occurs. Depending on the technique, fixation may involve sutures or temporary fixation devices that are later removed.
6. Closure
Small incisions are closed with sutures or clips. A light dressing may be applied. No drains are needed.
Recovery & Results
Recovery from endoscopic brow lift is relatively quick. Days 1-3: Mild to moderate swelling and bruising, primarily around the eyes. Sleep with head elevated. Forehead may feel tight or numb (temporary). Days 4-7: Swelling begins to subside. Sutures or clips removed around day 7-10. Most patients feel presentable for public by end of week 1. Week 2: Return to work and most normal activities. Avoid strenuous exercise. Minor bruising may still be fading. Weeks 3-4: Resume full activities including exercise. Final positioning of brows continues to settle. Months 2-6: Numbness or itching in the scalp gradually resolves, which can take up to six months. Final results visible by months 2-3.
You'll see improvement immediately, though final results emerge over 2-3 months as swelling resolves completely. Brows will be higher, forehead smoother, and you'll look more alert and rested. Results typically last 5-10 years, the aging process continues, but from a more youthful starting point. Many patients extend their results with periodic Botox treatments to the forehead and frown lines.
Frequently Asked Questions
Will a brow lift make me look surprised or startled?
No. Dr. Kurtzman's goal is natural-looking results that make you look refreshed, not surprised. The key is positioning the brow at its natural, youthful location, not artificially high. During consultation, we'll discuss exactly where your brows should sit for a natural appearance.
What's the difference between brow lift and Botox for forehead lines?
Botox relaxes muscles to temporarily reduce wrinkles (lasting 3-4 months) but cannot lift sagging brows. A brow lift surgically repositions descended brows and can remove excess skin, providing long-lasting lift that Botox cannot achieve. Many patients use Botox to maintain and extend their surgical results.
Can a brow lift be combined with other procedures?
Yes, and it often should be. Brow lift is frequently combined with blepharoplasty (eyelid surgery) for full upper face rejuvenation, or with facelift for total facial transformation. Addressing brows and eyelids together creates better harmony than either procedure alone.
Will I have visible scars?
With endoscopic technique, incisions are small (1-2cm) and placed within the hairline where they become virtually invisible once healed. Even patients with thinning hair typically have no visible scarring.
How long do brow lift results last?
Results typically last 5-10 years. The aging process continues, but from a more youthful starting point. Maintaining results with good sun protection, healthy lifestyle, and periodic non-surgical treatments like Botox helps extend longevity.
What if I need more lifting in the outer brow vs. the center?
Brow lift technique can be tailored to provide more lift in specific areas. The lateral (outer) brow often descends more than the medial (inner) brow, and Dr. Kurtzman tailors the procedure to address your specific pattern of descent.
