The procedure combines two goals in one surgery: liposuction removes unwanted fat from donor areas (typically the abdomen, flanks, back, or thighs), while strategic fat injection creates fuller, rounder, more lifted buttocks. This dual benefit, slimmer waist AND curvier buttocks, is why BBL has become one of the fastest-growing procedures in plastic surgery. The Procedure
BBL is a sophisticated procedure that requires artistic vision and careful technique. Safety is paramount, we follow all current guidelines so that fat is transferred safely:
1. Anesthesia & Preparation
BBL is performed under general anesthesia. You're positioned to allow access to both liposuction donor sites and the buttocks. Strategic planning determines exactly where fat will be harvested and how much is needed.
2. Fat Harvesting via Liposuction
Using tumescent liposuction, fat is carefully removed from donor areas, typically the abdomen, flanks, lower back, and/or thighs. The goal is to harvest enough fat while also sculpting these areas for a dramatic hourglass transformation.
3. Fat Processing & Purification
Harvested fat is processed to separate healthy fat cells from blood, oil, and fluid. The purified fat is prepared for transfer. Proper processing is critical for fat survival.
4. Safe Fat Injection
Fat is injected into the buttocks using specialized cannulas at multiple depths within the SUBCUTANEOUS tissue (above the muscle). Small amounts are placed in multiple passes to maximize blood supply and fat survival. We never inject into or below the gluteal muscle.
5. Artistic Sculpting
Fat placement is strategically distributed to create the desired shape, upper pole fullness, lateral projection, and overall roundness. The goal is natural curves that complement your body proportions.
6. Closure & Compression
Small liposuction incisions are closed. A compression garment is applied to donor areas. You'll receive a special BBL pillow and detailed instructions for sitting and sleeping positions.
Recovery & Results
BBL recovery requires strict adherence to post-operative instructions to maximize fat survival. Weeks 1-2: NO sitting directly on buttocks, this is critical for fat survival. Sleep on your stomach or side. Use a BBL pillow when you must sit (toileting, eating, driving). Compression garment worn on liposuction areas. Moderate discomfort in both donor sites and buttocks. Weeks 2-3: Continue avoiding direct sitting as much as possible. When sitting is necessary, use BBL pillow. May return to work if you can stand or use BBL pillow. Bruising and swelling continue to improve. Weeks 3-4: Can gradually resume careful sitting with pillow. Return to most light activities. Swelling in buttocks begins to resolve. Months 1-3: Resume normal sitting gradually. Return to exercise (avoid direct buttock exercises initially). Final shape emerges as swelling resolves. Months 3-6: Full results visible. Surviving fat is permanent.

Frequently Asked Questions
How much fat do I need for a BBL?
Most patients need 800-1200cc of fat injected into each buttock (1600-2400cc total) for noticeable enhancement. This means we typically need to liposuction 2-3+ liters of fat from donor areas to have enough after processing. During consultation, we'll assess your donor sites and determine if you have adequate fat. Very thin patients may not have enough fat for significant augmentation.
Why can't I sit after BBL?
Sitting puts direct pressure on the newly transferred fat, which can compress and kill fat cells before they establish blood supply, significantly reducing your results. For 2-3 weeks, you must avoid sitting directly on your buttocks. When sitting is necessary, use a BBL pillow that shifts weight to your thighs. This is the most important factor in maximizing fat survival.
Is BBL safe?
BBL is safe when performed by an experienced, Board-Certified plastic surgeon following current safety protocols. The key safety concern is fat embolism, which occurs when fat enters blood vessels. We prevent this by ONLY injecting fat into the subcutaneous tissue (above the muscle), using proper cannulas, and following the guidelines established by leading plastic surgery organizations.
How long do BBL results last?
Fat that survives the initial 3-month period becomes permanent living tissue. However, this fat behaves like fat elsewhere in your body, it will enlarge with weight gain and shrink with weight loss. Maintaining a stable weight is essential for preserving your results. Many patients enjoy their enhanced curves for decades.
What's the difference between BBL and buttock implants?
BBL uses your own fat for a natural look and feel, with the added benefit of slimming donor areas. Implants provide more predictable augmentation but can feel firmer and have higher complication rates (capsular contracture, implant displacement). Most patients prefer the natural results and dual contouring benefits of BBL.
Can BBL be combined with other procedures?
BBL is commonly combined with tummy tuck (when there's excess skin in addition to fat), breast surgery, or additional body contouring. However, the liposuction component of BBL already provides significant body sculpting. We'll discuss combination options during consultation based on your goals and safety considerations.
